Travel Vaccinationsglobal air travel

If you require any vaccinations relating to foreign travel you will need to complete the following travel questionnaire. These can also be obtained from the Surgery by visiting the reception desk.

You will need to complete one questionnaire for each individual travelling in your party. The questionnaire(s) will need to be returned a MINIMUM of 8 Weeks before you travel.

Please note: If you are taking a last minute break and are travelling in less than 6 weeks, we will be unable to provide you with a travel health service. In this instance the nearest independent (non-NHS) travel health service is in Boots Barnstaple.

It is important to return the completed questionnaire(s) as soon as possible - at least 8 weeks before you travel - as an appointment may be required with the practice nurse to receive any vaccinations you may need. These vaccines have to be ordered as they are not a stock vaccine. An appointment to receive the vaccinations needs to be at least 2 weeks before you travel to allow the vaccines to work.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
